Step 1:
Visit the District Social Welfare Office, and request a hard copy of the format of the application form for the scheme from the concerned authority.

In the application form, fill in all the mandatory fields, paste the passport-sized photograph (signed across), and attach all the (self-attested) mandatory documents.

Submit the duly filled and signed application form along with the documents to the
District Social Welfare Officer

Acquire the receipt/acknowledgment of the successful submission of the application form from the District Social Welfare Office.

On receipt of the application duly completed by the applicant, the DSWO shall consolidate the list and forward it to the District Level Sanctioning Committee.

shall admit/discharge any inmate with the approval of the District Social Welfare Officer concerned and the Deputy Director of Administration, Directorate of Social Welfare Jammu.
